Importance of Circulating Leptin and Adiponectin in the Causal Pathways Between Obesity and the Development of Colorectal Cancer in Japanese Men
 
  •  Using data from a large prospective cohort study of 44,271 Japanese men, we decomposed the total causal effects of obesity on the risk of colorectal cancer (CRC) into three path-specific components: (1) direct effects; (2) indirect effects that are mediated by circulating leptin and adiponectin; and (3) indirect effects that are not mediated by circulating leptin and adiponectin, but by hyperinsulinemia and chronic inflammation, represented by circulating C-peptide and C-reactive protein, respectively.
  • Our mediation analyses based on the multiple imputation indicated that there were indirect effects mediated by circulating leptin and adiponectin [risk ratio for body mass index (BMI) 25.0–27.4 kg/m2 was 1.29; 95% confidence interval, 0.98–1.69, and risk ratio for BMI ≥27.5 kg/m2 was 1.28; 95% confidence interval, 0.98–1.68], which may suggest a major role for these molecules in the pathway connecting obesity and CRC risk.
  • Similar results were obtained using the inverse probability weighting method with design-based sampling probabilities (risk ratio for BMI 25.0–27.4 kg/m2 was 1.26; 95% confidence interval, 1.11–1.40, and risk ratio for BMI ≥27.5 kg/m2 was 1.49; 95% confidence interval, 1.06–1.89).
The Association Between COVID-19-related Discrimination and Probable Post-traumatic Stress Disorder Among Patients With COVID-19 in Sapporo, Japan
 
  • We aimed to investigate the relationship between coronavirus disease 2019 (COVID-19)-related discrimination and probable post-traumatic stress disorder (PTSD).
  • The adjusted odds ratio of COVID-19-related discrimination for probable PTSD was 4.73.
  • The population attributable fraction of probable PTSD attributable to COVID-19-related discrimination among COVID-19 patients was estimated to be 23.4%.
  • Mitigating discrimination could be helpful to attenuate PTSD in future pandemics.
Paternal Involvement in Childcare and Housework and Mothers’ Spanking Behavior: The Japanese Longitudinal Survey of Newborns in the 21st Century
 
  • The association between fathers’ involvement in childcare and housework and mothers’ use of spanking as a disciplinary measure was assessed among 16,373 mothers in Japan.
  • Mothers are less likely to use spanking as a disciplinary measure when fathers actively participated in housework.
  • Active involvement of fathers in childcare was associated with fewer instances of mothers’ spanking their children, when fathers worked less than 50 hours/week.

Perceived Familial Financial Insecurity and Obesity Among Korean Adolescents During the COVID-19 Pandemic
 
  • Socioeconomic disparity on health may have accelerated during the early phase of the coronavirus disease 2019 (COVID-19) pandemic.
  • We examined the association between perceived financial insecurity and obesity using a series of nationwide surveys of Korean adolescents.
  • Out of 106,979 adolescents aged 12–18 years, 70.5% reported experiencing the COVID-19 pandemic-induced financial deterioration.
  • Subjective socioeconomic status and family-level financial deterioration due to pandemic were independently and additively associated with obesity.

Rare Event Approximation Between Subdistribution Hazard Ratio and Cause-specific Hazard Ratio in Survival Analysis With Competing Risks

  • The difference in the values of two distinct hazard ratios, namely the subdistribution hazard ratio and cause-specific hazard ratio, in the context of competing risks, can be addressed by rare event assumptions and a survival ratio.
  • An approximation formula, grounded in the survival ratio, is derived to facilitate the conversion of one hazard ratio into the other.
  • The approximation formula is effective in calculation of a subdistribution hazard ratio from published information and sample size estimation while considering the presence of competing risks.

Regional Variation in End-of-life Care Just Before Death Among the Oldest Old in Japan: A Descriptive Study

  • The regional variations in life-sustaining treatment among patients aged ≥85 years in Japan were described using national representative claims data.
  • The variation in the use of cardiopulmonary resuscitation, mechanical ventilation, and admissions in the last 7 days of life was observed.

Suicide Risk Among Patients With Cancer by Sex in Japan: A Population-based Study
 
  • We estimated the risk of suicide among patients with cancer by sex in Japan using standardized mortality ratios (SMRs).
  • The risk of suicide among patients with cancer was equally high for both sexes compared to the general population.
  • Suicide risk was higher for both men and women with advanced-stage cancer or poor prognosis.
  • Men with a good prognosis had little difference in risk from the general population, while men with a poor prognosis had a high SMR.
  • Women had a smaller difference in risk in the prognosis groups than men.
 
The Out-of-pocket Expenses of People With Tinnitus in Europe
 
  • The estimates for the out-of-pocket expenses for people with tinnitus reached the record figure of over 17 billion euros.

GWAS Meta-analysis of Kidney Function Traits in Japanese Populations
 
  • Genetic epidemiological evidence for the kidney function traits in East Asian populations, including Japanese, remain still relatively unclarified.
  • We conducted a genome-wide association study (GWAS) meta-analyses of kidney traits by leveraging the GWAS summary data of the representative large genome cohort studies with about 200,000 Japanese participants (n=202,406 for estimated glomerular filtration rate and n=200,845 for serum creatinine [SCr]).
  • In the present GWAS meta-analysis, we identified one novel locus (more than 1Mb distant from known loci) with one variant (CD36 rs146148222 on chromosome 7) for SCr.
  • A reduction in fatty acid oxidation (FAO) in chronic kidney disease has an important role in kidney fibrosis, and CD36 is considered to play important roles in FAO through its function in the mitochondrial transfer of fatty acids.
  • The present GWAS meta-analysis of largest genome cohort studies in Japanese provided some original genomic loci associated with kidney function in Japanese, which may contribute to the possible personalized prevention of kidney diseases in the near future.
 
Long Working Hours, Work-life Imbalance, and Poor Mental Health: A Cross-sectional Mediation Analysis Based on the Sixth Korean Working Conditions Survey, 2020–2021
 
  • We explored the association between working hours, work-life imbalance (WLI), and poor mental health.
  • Working hours per week were classified into the following categories: “35–40 hours,” “41–48 hours,” “49–54 hours,” and “≥55 hours.”
  • Working >40 hours per week was associated with higher WLI levels than working 35–40 hours per week.
  • Working >40 hours per week was positively associated with poor mental health compared to working 35–40 hours per week.

Association Between Temporary Employment and Current Smoking and Change in Smoking Behaviors: A Prospective Cohort Study From South Korea (2009–2018)
 
  • We studied the association between temporary employment and smoking behaviors.

  • Workers with temporary employment were found to be more likely to be current smokers compared to those with regular employment.

  • Workers with temporary employment were associated with a decreased likelihood of smoking cessation compared to those with regular employment.

  • In both men and women, persistent temporary employment was negatively associated with smoking cessation compared to maintaining regular employment.

Development and Validation of Claims-based Algorithms for Identifying Hospitalized Patients With COVID-19 and Their Severity in 2020 and 2021
 
  • We evaluated the validity of claims-based algorithms for coronavirus disease 2019 hospitalization, moderate or higher status, and severe status during four periods: the first and second halves of 2020 and the first and second halves of 2021.

  • The diagnosis-only algorithm for hospitalization consistently showed high sensitivities (90.4–94.9%) and low PPVs (9.3–19.4%) throughout the four periods.

  • Sensitivities and PPVs of algorithms, consisting of both diagnosis codes and procedure codes, varied over time, but high sensitivities and PPVs were observed during the following periods: 93.9% and 97.1% for hospitalization (Jan–Jun 2021), 90.4% and 87.5% for moderate or higher status (Jul–Dec 2021), and 92.3% and 85.7% for severe status (Jul–Dec 2020).

  • Almost all algorithms had specificities and negative predictive values of approximately 99% throughout the study period.

Changes in Place of Death Among Patients With Dementia During the COVID-19 Pandemic in Japan: A Time-series Analysis
 
  • Trends in deaths were examined among individuals with dementia in Japan between December 30, 2019, and January 29, 2023.

  • Overall, there was a decrease in deaths of people with dementia at the beginning of 2020, followed by excess deaths in December 2020.

  • Excess deaths in homes, healthcare facilities, and nursing homes began in October 2020, December 2020, and March 2021, respectively.

  • Societal policies, precautions during the pandemic, and changes in the healthcare environment might have affected the preferred place of death.

Metabolomics Profiles Alterations in Cigarette Smokers and Heated Tobacco Product Users
 
  • Metabolomics study of over 9,900 Japanese participants reveals distinct metabolite profiles in cigarette smokers compared to never smokers.

  • Heated tobacco product (HTP) users showed metabolite profiles closer to cigarette smokers than never smokers, indicating similar health risks.

  • Biomarkers involved in glutamate (Glu) metabolism were associated with both cigarette smoking and HTP use.

  • Glu pathway activation in smokers and HTP users may contribute to cardiovascular diseases, supporting the link between smoking and atherosclerosis.

Associations of Individual Characteristics and Socioeconomic Status With Heated Tobacco Product Harmfulness Perceptions in Japan: A Nationwide Cross-sectional Study (INFORM Study 2020)
 
  • This study revealed the factors related to heated tobacco product (HTP) harmfulness perception, including trusted sources of cancer information, among both tobacco users and tobacco non-users.

  • The results showed that being male, aged under 39 years, and having lower education (less than college) were associated with a perception of HTP as less harmful only among tobacco non-users.

  • Moreover, the use of HTPs in tobacco users was associated with harmfulness perception.

Coffee and Green Tea Consumption With the Risk of COVID-19 Among the Vaccine Recipients in Japan: A Prospective Study
 
  • Although coffee and green tea have been suggested to have immunoprotective effects, epidemiological evidence is scarce on the association of coffee and green tea consumption with the risk of coronavirus disease 2019 (COVID-19).

  • In the current study, higher consumption of coffee was associated with an increased risk of COVID-19 among the staff of a large referral hospital in Japan who received the mRNA vaccine.

  • There was no evidence of a significant association of green tea consumption with the risk of COVID-19.

Validity of Self-reported Helicobacter pylori Eradication Treatment From Questionnaire and Interview Surveys of the JPHC-NEXT Study: Comparison With Prescription History From Insurance Claims Data
 
  • Self-reports of H. pylori eradication showed high sensitivity, high specificity, and good agreement with the insurance claim prescriptions data.

  • Self-reports, whether by self-administered questionnaires or face-to-face interviews, were reliable methods of ascertaining H. pylori eradication.

Epidemiological Analysis of Legionella Pneumonia in Japan: A National Inpatient Database Study
 
  • We analyzed the Japanese Diagnosis Procedure Combination database to determine the epidemiology of Legionella pneumonia.

Association Between Birth Weight and Prevalence of Cardiovascular Disease and Other Lifestyle-related Diseases Among the Japanese Population: The JPHC-NEXT Study
 
  • In a large Japanese population-based cohort, higher prevalence of cardiovascular disease was observed among those with low birthweight.
  • Similarly, higher prevalence of hypertension and diabetes were observed among those with low birthweight.

The Association Between the Maternal Pre-pregnancy Platelet Count and Fecundability in Mainland China: A Population-based Cohort Study
 
  • Maternal pre-pregnancy platelet count (PC) is associated with female fecundability. Further classification of PC levels may deepen our understanding of the early warnings and significance of female fecundability.
  • The first (<177.00 ×109/L) and second quintile (177.00-207.99 ×109/L) of PC showed slightly increased fecundability, while higher quintals (Q4: 236.00-271.99 ×109/L; Q5: ≥272.00 ×109/L) were related to reduction of fecundability compared with the third quintile of PC (208.00-235.99 ×109/L). In the first quintiles (<177.00×109/L), only 20.93% women had PC below 129.94×109/L.
  • An inverse-U-shaped association was consistently observed among women such that the lower PC of normal range (<118.03×109/L) and higher PC (>223.06×109/L) were associated with the risk of reduced female fecundability (P for non-linearity < 0.01).

Impact of the COVID-19 Pandemic on Cancer Death Locations in Japan: An Analysis of Excess Mortality Through February 2023
 
  • The novel coronavirus disease 2019 (COVID-19) pandemic influenced where cancer patients in Japan passed away.
  • From 2018 to 2023, the percentage of deaths in medical institutions declined from 83.3% to 70.1%, while deaths at home increased from 12.1% to 22.9%.
  • There was no clear evidence suggesting an increase in excess cancer deaths associated with the pandemic.
  • Factors such as restrictions on healthcare visitations might have contributed to the shift.
  • The decrease in cancer diagnoses was transient and might not have been significant enough to result in excess cancer deaths.

Trends in the Stage Distribution of Colorectal Cancer During the COVID-19 Pandemic in Japan: A Nationwide Hospital-claims Data Analysis
 
  • We investigated the trend of the stage distribution of colorectal cancer during the coronavirus disease 2019 (COVID-19) pandemic using the nationwide hospital-based claims database in Japan.
  • During the COVID-19 pandemic in 2020, the percentage of early-stage colorectal cancers tended to decrease in July after the state of emergency was declared in April and May.
  • The impact of the COVID-19 pandemic on the distribution of colorectal cancer stage lasted for a few months after July and then became smaller.

Smoking is Associated With Impaired Long-term Quality of Life in Elderly People: A 22-year Cohort Study in NIPPON-DATA 90

Effectiveness of BNT162b2 Vaccine Against Symptomatic SARS-CoV-2 Infection in Children Aged 5–11 Years in Japan During Omicron Variant Predominate Periods

  • Although several genome-wide association studies (GWASs) on folate metabolisms have been conducted, few studies have examined the interaction between folate-related single-nucleotide polymorphisms and lifestyles.
  • We conducted the GWAS of plasma homocysteine (Hcy), folic acid (FA) and vitamin B12 (VB12) levels using the data of 2,263 participants from four study sites of the J-MICC Study in the discovery phase, and data of 573 participants from the Yakumo Study were analyzed in the replication phase.
  • For Hcy, MTHFR locus on chromosome (chr) 1, Nox4 on chr11, CHMP1A on chr 16, and DPEP1 on chr 16 reached GWAS significance (P < 5×10-8). MTHFR was also associated with FA, and FUT2 on chr 19 was associated with VB12.
  • The analyses on gene-environment interactions revealed significant interactions of MTHFR C677T with drinking and physical activity on blood folate metabolites, suggesting the possibility of future personalized cardiovascular disease prevention.

Validity of Using Japanese Administrative Data to Identify Inpatients With Acute Pulmonary Embolism: Referencing the COMMAND VTE Registry
 
  • We evaluated the validity of algorithms for the identification of acute pulmonary embolism (PE) patients in Japanese diagnosis procedure combination (DPC) data referencing registry data.
  • Diagnosis information in the discharge abstract data from the Japanese DPC database enabled us to detect more than 90% of acute PE inpatients.
  • When restricted to symptomatic cases of PE, 94.6% of the patients were identified by the diagnosis-based algorithm.
  • The diagnosis-based algorithm may be a relatively sensitive method for identifying acute PE patients in the Japanese DPC database.

Development and Validation of Prediction Models for the 5-year Risk of Type 2 Diabetes in a Japanese Population: Japan Public Health Center-based Prospective (JPHC) Diabetes Study
 
  • To the best of our knowledge, these are the first diabetes risk scores developed and validated using a nationwide population of Japan to predict the 5-year incidence of type 2 diabetes.
  • In this prospective study, the diabetes risk scores used both non-invasive and invasive predictors to predict the 5-year incidence of type 2 diabetes.
  • The area under the receiver operating characteristic curve was 0.643 for the non-invasive risk model, 0.786 for the invasive risk model with glycated hemoglobin (HbA1c) but not fasting plasma glucose (FPG), and 0.845 for the invasive risk model with HbA1c and FPG in a development cohort. The discriminative ability of each model was confirmed using external validation datasets. The invasive risk model with only HbA1c was well-calibrated in the validation cohort.
Impact of Radiofrequency Exposure From Mobile Phones on the Risk of Developing Brain Tumors in Korean and Japanese Adolescents: A MOBI-Kids Case-control Study
 
  • Second generation (2G) and personal handy-phone system were used in Japan mainly in the 1990's, where third generation (3G) was adopted in Korea in the mid-2000s. On average, output power from 2G phones was higher than from 3G phones.
  • The international MOBI-Kids case-control study provided no evidence of a causal association between wireless phone use (and electromagnetic field exposure) and brain tumors in young people.

BMI and Cardiometabolic Traits in Japanese: A Mendelian Randomization Study
 
  • The causal relationships between body mass index (BMI) and cardiometabolic traits in Japanese were assessed using the Mendelian randomization (MR) approach in a sample of 14,083 Japanese.
  • In individual-level MR analyses, predicted BMI was not significantly associated with any cardiovascular traits in the Japanese population.
  • Sensitivity analyses based on two-sample MR analyses of 173,430 Japanese subjects showed that BMI is associated with a variety of cardiometabolic traits.
  • Even among the Japanese, an East Asian population with low levels of obesity, higher BMI could be causally associated with a variety of cardiometabolic traits, in line with the results of European studies.
  • The causality of these associations should be clarified in future studies with larger populations.

Dose-response Associations of Physical Activity and Sitting Time With All-cause Mortality in Older Japanese Adults

  • A significant inverse non-linear dose-response relationship was identified between moderate-to-vigorous PA (MVPA) and all-cause mortality.

Association Between Advanced Airway Management With Adrenaline Injection and Prognosis in Adult Patients With Asystole Asphyxia Out-of-hospital Cardiac Arrest

  • Advanced airway management (AAM) for patients with asphyxia out-of-hospital cardiac arrest (OHCA) with adrenaline injection was associated with better 1-month survival rates and return of spontaneous circulation.
  • However, AAM was not associated with favorable neurological and functional outcomes.
  • Especially, in older patients, it is important to understand the effect of AAM for asphyxia OHCA in asystole.

Variance Estimation for Logistic Regression in Case-cohort Studies
 
  • In case-cohort studies, the logistic regression analysis has been a standard method in current practice, and it enables effective estimation of risk ratios with adjustment for potential confounding factors.
  • The robust variance estimator is widely used for assessing the standard error of the risk ratio estimator, but we showed it has certain bias under general settings (ie, the resultant confidence intervals and P-values are possibly inaccurate).
  • To address the invalid statistical inference problem, we provide an alternative bootstrap-based valid variance estimator. We also provide a R package bootcc (https://github.com/nomahi/bootcc) for implementing the bootstrap inference method using simple commands.
  • Through simulation studies, the bootstrap method consistently provided more precise confidence intervals compared with those provided by the robust variance method, while retaining adequate coverage probabilities.
  • The robust variance estimator has certain bias, and inadequate conclusions might be deduced from the resultant statistical analyses.The valid bootstrap procedure will be an alternative effective method in practice to provide accurate evidence.
