Highlights
- Older Japanese adults exhibit the highest drowning mortality rates globally, potentially linked to the cultural practice of in-home bathing.
- Using death certificates from 1995 to 2020, we calculated national age-adjusted mortality rates (AMRs) and prefecture-specific age-standardized mortality ratios for unintentional drowning and submersion in bathtubs (ICD-10 code W65).
- Persistently elevated AMRs indicated that the increasing mortality associated with W65 is not solely attributable to population aging.
- Our findings indicated a negative association between the number of nursing care facilities and users and mortality coded as W65 at the prefectural level.
