  • The regional variations in life-sustaining treatment among patients aged ≥85 years in Japan were described using national representative claims data.
  • The variation in the use of cardiopulmonary resuscitation, mechanical ventilation, and admissions in the last 7 days of life was observed.
  • Further research on the factors related to variations and the challenge toward equalization and quality improvement of care is needed.
