Highlights

• We estimated the risk of suicide among patients with cancer by sex in Japan using standardized mortality ratios (SMRs).
• The risk of suicide among patients with cancer was equally high for both sexes compared to the general population.
• Suicide risk was higher for both men and women with advanced-stage cancer or poor prognosis.
• Men with a good prognosis had little difference in risk from the general population, while men with a poor prognosis had a high SMR.
• Women had a smaller difference in risk in the prognosis groups than men.
